Text Box: Remember: You must get a signed letter from your department on official letterhead. It must have an original signature from a department head. You must complete and sign the individual affidavit. You must provide a copy of your id. We will not sell or release any guns until these forms are completed. To assist individual law enforcement officers, we have provided the necessary forms on-line.  You must have both forms completed.  It will be necessary that one is completed by your department on official letterhead. The other will be completed by yourself.

Each downloaded form includes a page that can be modified and another page to simply print and fill in the blanks.
